Now let me tell you about how to fill adsense account application form. First go to  adsense  it asks proceed to existing account or create a Google account( Gmail account). You can select one of the two options.

If you have Google account then click on yes proceed to Google account sign in. You can see in image below.

It will show you the next image shown in below, you should give the details in the appropriate places. Give your URL address of your website. And below select content language of your website. Then click on continue as shown in image below.

Next you get the another form in this you should give country, time zone, account type.Payee name as like in your bank account, address including with your street address, phone number and give few more details like shown in image below.

Then click on submit my account.
